TidalHealth Peninsula Regional (formally PRMC) undertook an expansion project which included major additions and renovations to the existing 650,269 sf building and campus. Additions totaling 214,095 sf included a new outpatient entrance, emergency room, critical care unit, cancer center, and new heart center atrium in addition to 78,840 sf in other renovations. The project also included a new heliport and five-story parking garage. Planning efforts included the closure of three city streets, partial closure of two others, and the relocation of West Vine Street. Site circulation was vastly improved with new parking facilities, roadways/traffic patterns, drop-off areas, and sidewalks.

New landscaping throughout the campus created a more relaxed and friendly campus environment. Green technologies were implemented to reduce the overall impact on the environment, including a pervious asphalt paved parking lot and pervious concrete paved employee parking lot (the first of its type on the Eastern Shore). The new pervious concrete parking lot won the Sustainability Award in Concrete awarded by the Maryland Chapter of ACI (American Concrete Institute). Services also included consolidation and relocation of major public utilities. Multiple entrance improvements help disperse and upgrade the circulation and parking, create safe pedestrian circulation, and add landscaping to create a new green campus environment.
